Job Title:
CARER ASSESSMENT COORDINATOR
Wigan and Leigh Carers Centre provides practical and emotional support to carers across the Borough. The Centre is now recruiting the above fulltime posts, to support carers through the carer assessment process
The appropriate candidates will have a background in Health and Social Care, be conversant with issues affecting carers and their rights, together with some local knowledge.
Contract Period:
Fixed term period of 12 months
Hours:
35 hours per week
Salary:
£19,950 per annum

Overview:
The appropriate candidates will have a background in Health and Social Care, be conversant with issues affecting carers and their rights, together with some local knowledge.
- To undertake holistic assessments of Carers needs
- To support Carers, through the assessment process. This will be achieved in a variety of ways, for example, visiting Carers in their homes or elsewhere as deemed appropriate.
- This role is based predominately in the community with home visits being a primary method of contact.
- To assess Carers support needs, complete appropriate assessment forms, computerised records and other associated administrative tasks.
- Completion of Carer’s Support Plans, as appropriate
- To use an asset based approach considering all the services available to meet the Carer’s assessed needs.
- To signpost carers to appropriate services available to them.
- To Liaise with colleagues in Wigan Council as required
